Refactor forward extremities (#1556)

* Add resolve-state helper

* Tweaks

* Refactor forward extremities, again

* Tweaks

* Minor optimisation

* Make path a bit clearer

* Only process state/membership if forward extremities have changed

* Usage comments in resolve-state
